We have a problem Huston. Rosie and I were not given a card for our first dose but were on the 2nd. The person said we can fill out the card ourselves for the first. Our problem is the lot no. of the first shot. We have contacted everyone involved and still can't get an answer. Does anyone have a card with a lot no. for 3/4 in Crystal River Square, Pfizer vaccine? Thanks.
You have been talking to the wrong person. Every vaccination site is required to keep a record of all vaccinations. That's how you were able to get your second shot. Many people have lost their vaccination cards and have gotten replacements printed out. The drug stores do this routinely. Of course, you are dealing with the county where not so many people know what they are talking about. Go to the Health Department in person and don't give up until they print you our replacement cards.
We didn't get a notice for our 2nd shots just showed up @ the place, 3 weeks after the first ones, and had to show an email about our 1st appointments. We asked there, called the state health department and the county health department for our 1st record. The nurse that gave us the 2nd dose and card said we could fill the card ourselves and we would if we can get the lot no.
You will need to go in person to the Health Department in order to get that information. They should have it.
On the other hand, it is not clear how useful that card will be even if fully filled out. I doubt that the US is going to have vaccine passports. The idea is just not well accepted independent of grandstanding by politicians and pundits. The only time I can forsee that card being useful is if you decide to travel to another country.
Some countries will be adopting vaccine passports regardless of what the US does. They may require negative Covid test results as well as proof of vaccination.
Doc Stressor was right as usual. Went to the health department this morning and they had records of our vaccinations and filled out our cards. Thanks Lee. This was after many phone calls to the state, county and the feds. BTW it is Houston.
Got the first Pfizer vaccine, second dose coming up in a little over a week. It’s been a long year, will be glad to see things getting back to somewhat normal. Hopefully!

Got a call yesterday from primary Dr to come in for flu shot. Last time I went in he wanted to give me shingles shot. I told him I never had chicken pox that I didnt need it. He didn't give it to me.
