I learned early on after hearing others to ALWAYS mute my side until I want to speak. I do require my employees to identify themselves each and every time they speak. They are well conditioned by now.

Test your mute button on your phone...some mute but you can still just barely hear the person.

Participants change depending on the meeting. I recognize my direct reports voices but their employees only participate when needed so I may not recognize them. I try to have people from each department participate who are not direct reports so they feel included and they get to interact with department heads they may not deal with day to day.
